Network Providers
Finding a Provider
You will be asked WHAT you wish to see the provider about. That way we can match you with someone with expertise in your area of concern. You will be asked WHERE you would like to see the provider. You name the town, and we will do our best to match you with someone in that area. Our goal is to find you a provider within 20 minutes or 20 miles of home or work. Typically, your provider will be within 5 miles from your home or work. Just call 1-800-300-0628 to access your Saint Barnabas Employee Assistance Program. Locating a Provider Your Employee Assistance Program offers services throughout the United States and abroad. By calling your EAP, you will be matched with a provider in an office close to your home or work. For additional information and to be matched with a provider, please call your EAP first at 1-800-300-0628. Provider Credentialing All providers are credentialed with a license (or certification) by their state. Most of our providers are licensed social workers. We evaluate providers carefully, to ensure they meet our high standards for education, training, experience, licensure, and skill. To contact your EAP and be matched with a provider, simply call 1-800-300-0628. A clinician will answer the phone and speak with you 24 hours each day, 7 days each week. This clinician will help match you with a provider who has expertise in your area of concern. Billing Current Providers wishing to contact our Billing Department should reach the SBBHN EAP at eapbilling@sbhcs.com. Providers Relations Current Providers wishing to contact our Provider Relations Department should reach the SBBHN EAP at network@sbhcs.com. How to Become a Provider To become a provider with the Saint Barnabas Employee Assistance Program (SB EAP), you will need to complete our application and provide us with the specified documentation.
